You can use SystmOnline, a simple and secure way to access a range of NHS services on your smartphone, tablet or PC / Laptop.

How to register for SystmOnline – To register for SystmOnline, you can complete the registration form by clicking here

You will need to provide photo identification (e.g. driving licence or passport). If you are unable to provide photo ID we can also accept 2 forms of proof of address eg bank statement or utility bill.

We will then issue you a username and password. Once logged in, we suggest that you change the password to something you can easily remember!

If you’d prefer to register in person, you can attend either Carterknowle or Dore surgery and bring your photo ID and we can register you at reception.

What services are available via Online Services – 

Order repeat prescriptions – See your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to.

Proxy Access

Proxy Access Requests

    • We are updating our process for proxy access to come into line with NHS guidance (https://www.england.nhs.uk/long-read/proxy-access/ ) regarding this.
    • All new requests for proxy access will require a signed consent form which can be collected from the practice or accessed by clicking the link –  Proxyconsentform , signed either by the patient or on behalf of (if the patient cannot sign i.e. is a child or has a medical issue that prevents them from giving consent). Please note that for children aged 11 and upwards, unless there are medical reasons why they cannot give consent, they would be expected to have read and signed the form themselves (and a parent signing on their behalf would not be acceptable).

    • When attending the practice, we will also need to verify ID.For the DONOR (i.e. the person whose record will be accessed) we ideally need to see them in person and/or see a form of ID or a Birth Certificate (i.e. for children who don’t have any government photo ID yet).For the APPLICANT (i.e. the person(s) who are requesting access) they must attend in person with a valid form of government issued photo ID i.e. a driving licence, passport etc).
    • For situations where access is requested based on a legal agreement such as Lasting Power or Attorney, or the applicant is acting as a Court Appointed Deputy on behalf of the patient, the practice may request proof of this if it does not already possess it.
    • Upon receipt of a signed consent form and having verified the ID as above, we will review and grant/decline access accordingly. The level of access granted is determined by the practice.
