 This feature will hopefully provide a more streamlined way for patients to contact us.

This form can be used to put requests in before the practice phonelines (this includes on the day requests i.e. requests for same day appointments or routine appointments), please be mindful however that the practice staff are in from 0800 and your request will likely be reviewed then and not before. If you needed immediate medical attention before we are open, you should be phoning 111 or 999.

  Our telephone triage systems will remain online throughout, and if you still wish to ring us, you may do so on the day (the telephone and online systems work alongside each other).

We would ask that all same day appointment requests come in as early as possible to ensure we have enough time to review your request and offer you an appointment.

   If we can’t offer you an appointment, we may signpost patients to other services such as the walk-in centre or 111
