If you are an asthmatic patient, we wanted to ensure you have as much information as possible on managing and understanding your condition.

If you would like to discuss this with our team, please arrange an appointment, you can contact us via Accurx, or call the team on 0114 229 1686.
If you are due your annual (Birthday Medical) review, you will receive a questionnaire asking further information about your Asthma. The team will then review your response and advise whether an appointment is necessary and follow you up with the relevant outcome. Face to face appointments are ideal but we realise that this is not always convenient for you so can also offer telephone appointments to suit your preferences.
